(2024-09-05) Bailo, Vivian de Souza; Batista, André Mariano; http://lattes.cnpq.br/5615914349535394; http://lattes.cnpq.br/4089579627319222
The Mandatory Supervised Internship (referred as ESO in portuguese) in Veterinary Medicine was conducted in the fields of Veterinary Anesthesiology and Equine Clinic and Surgery from April 1 to 30, 2024, at the FZEA/USP-Pirassununga Veterinary Hospital, under the supervision of Prof. Dr. Adriano Bonfim Carregaro; from May 1 to 31, 2024, at the Clínica Veterinária Guadalupe, under the supervision of M.V. Guilherme Alberto Machado; and from June 1 to 30, 2024, at the FMVZ/UNESP-Botucatu Veterinary Hospital, under the supervision of Prof. Dr. Antônio José de Araújo Aguiar. The activities were coordinated by Prof. Dr. André Mariano Batista and totaled 424 hours. This internship provided exposure to the anesthetic routines at USP and UNESP hospitals and the general routine at Clínica Guadalupe. The work aimed to report a case of multimodal general anesthesia for exploratory celiotomy in an equine patient with acute abdomen syndrome. The procedure was performed on a 6-year-old female horse with a history of severe abdominal pain unresponsive to clinical treatment. The acute abdomen syndrome was caused by a fecalith in the descending colon, which was removed during the emergency surgery. Anesthesia was induced with ketamine (2.2 mg/kg) and midazolam (0.1 mg/kg), maintained with isoflurane (2% vol.) in 100% oxygen with mechanical ventilation, and a continuous infusion of lidocaine at 3 mg/kg/h. Due to severe hypotension recorded by the multiparameter monitor, dobutamine, norepinephrine, and hypertonic solution were used to correct this alteration, and fluid therapy with Ringer Lactate and NaCl 0.9% was combined to maintain blood volume. Blood gas analisys monitoring was crucial for assessing the patient's ventilatory function and the need for calcium, potassium, and sodium bicarbonate replacement. With the stabilization maneuvers performed, the patient remained stable throughout the procedure. Post-surgery, the animal experienced a prolonged and poor-quality anesthetic recovery, likely due to systemic changes related to the clinical condition. Thus, equine anesthesia in cases of colic is challenging and requires the anesthetist to have extensive knowledge and practice in physiology, pathophysiology, and pharmacology to achieve satisfactory results.

The Mandatory Supervised Internship (MSI), a required component of the undergraduate Veterinary Medicine program at the Federal Rural University of Pernambuco (UFRPE), consists of a total workload of 420 hours and is considered an essential activity for the professional qualification of students in their final semester. During the MSI, students have the opportunity to apply the knowledge acquired throughout their studies, preparing them for practical professional activities. This report aims to describe the MSI carried out between April 1st and June 14th, 2024, divided between the Ospedale Veterinario Universitario Didattico at the University of Naples and the Ospedale Veterinario Universitario Didattico at the University of Padua, both located in Italy. The report highlights a clinical case of ovariectomy in a domestic dog (Canis lupus familiaris), utilizing Zenalpha medication, composed by medetomidine hydrochloride (0,5 mg, an alpha-2 agonist) and vatinoxan hydrochloride (10 mg, an alpha-2 antagonist), combined with inhalational anesthesia using isoflurane and analgesia with methadone, complemented by subcutaneous infiltration of bupivacaine at the surgical incision site after the procedure. It is concluded that the combination of multimodal analgesia using Zenalpha and methadone, along with inhalational anesthesia with isoflurane and infiltrative bupivacaine, represents an effective strategy to provide analgesia during and after ovariectomy in female dogs, ensuring greater comfort during the perioperative period.

The Mandatory Supervised Internship (ESO), a mandatory subject of the Bachelor's Degree in Veterinary Medicine at the Federal Rural University of Pernambuco (UFRPE), has a workload of 420 hours and can be characterized as a professional qualification activity for the student in the last period of the course, applying their acquired knowledge and qualifying them for professional practice. This report is divided into two chapters. The first chapter describes all the activities that were monitored during the ESO period by the student, between the period of April 1, 2024 to June 14, 2024, at the Caminho dos Bichos Veterinary Clinic, located in the Espinheiro neighborhood, Recife/PE. The second chapter will report the case of a domestic feline that underwent anesthesia and surgery called cholecystectomy, in which the ultrasound-guided locoregional blockade of the quadratus lumborum (QL) was performed, using the combination of bupivacaine (local anesthetic), dexmedetomidine (alpha 2 agonist) and dexamethasone (corticosteroid). Locoregional anesthesia has been widely used in veterinary medicine, as it allows the reduction of general anesthetics for a favorable intraoperative period, in addition to reducing the use of analgesics in the postoperative period and their side effects. In this reported case, according to the monitored parameters, no pain was observed, as assessed by the UNESP-Botucatu multidimensional scale. Ultrasonography ensured that the technique was performed successfully, anatomically identifying the muscles. Thus, the blockade can be performed safely, being effective for celiotomy and cholecystectomy.

The Mandatory Supervised Internship (ESO), a mandatory subject of the last period of the Undergraduate course in Veterinary Medicine at UFRPE, was carried out accompanying the Veterinary Doctor (M.V.) MSc Thaygo Marçal da Mota at several clinics in the Metropolitan Region of Recife in the period 02/ 10/2023 to 01/19/2024, from Monday to Friday, totaling 420 hours. During the ESO period there was guidance from Prof. Dr. Moacir Bezerra de Andrade, associate professor at UFRPE. ESO offers students the autonomy to choose to intern in which area they want to work in when they graduate, or which they feel more affinity with, or also to try out a new area in which they did not have as many opportunities during their degree. Therefore, the objective of this work was to report the activities carried out during ESO, as well as to describe a technique for anesthetic blockade of the auriculotemporal, caudal auricular and greater auricular nerves with the aid of the neurolocalizer and the ultrasound device, in an animal that underwent an ear pinna otohematoma correction procedure, aiming for greater trans-surgical and immediate post-surgical comfort for the patient and greater safety during the procedure. The patient was monitored throughout the procedure, as well as the effectiveness of the anesthetic block. It is concluded that these types of associated techniques are extremely effective and safe, generating good results during and after surgery, with the animal waking up calmly without signs of pain.
