When it comes to specialized services in Primary and Medium Complexity Care in small municipalities, many are noticeable. difficulties related to managerial issues, costs and required technological density. Thus, the present study evaluated the difficulties encountered by the Municipal Management in providing emergency and outpatient medical care to users of a Mixed Unit in the city of Lagoa do Carro/PE and its Basic Health Units. Faced with the need for specialized medical care, individual attention and qualified professionals in the most diverse areas is essential. The lack of a more specific service, due to the scarcity of equipment and qualified personnel, makes service in the city unfeasible, with the need for transfer to other units in the State. Through a case study, the research identified aggravation and complications in the patient's clinical condition, as there is no specialized care needed to identify the disease that the patient developed within the municipality itself. The study sought to identify ways for the Municipal administration to be able to meet these users' needs within the municipality and thus minimize difficulties faced by users of the municipality's public health system. Possibilities for future studies are also pointed out.
